Are there any differences in difficulty between the American and Japanese versions? I was playing the American one, but I stopped because I don't want to ruin the game with a possibly-inferior version.

I haven't noticed any difficulty differences. Maybe you take more damage in one of them but I couldn't say because I don't get hit much. If you like Streets of Desolation as the stage 2 song then play the Japanese one.
